Social Services Assistant
Alpine Breeze Health and Wellness is Seeking a full time Social Services Assistant
Duties Include:
- Enrich lives as a nursing center Social Services Assistant where you will work with residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs.
- The goal is to provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided under the leadership of the Social Services Director.
- Collaborate with the interdisciplinary team to promote and protect resident rights and well-being. This includes educating staff on the role of social services.
- Participate in the development of a written, interdisciplinary plan of care for each resident to address social service needs.
- Hold meetings with residents, families, and significant others as requested and to provide support and information to improve quality of life.
- Perform resident discharge planning activities such as family/ resident education, nursing center documentation and make referrals to appropriate agencies and suppliers. Establish relationships and maintain contact and referral flow with community-based agencies/services.
- Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our resident population.
- Ensure residents get to needed physician appointments by driving them in facility van.
Qualifications:
- High school diploma required
- Associate degree or higher in social services related field preferred.
- Minimum of two years' experience in long-term care.
- Must have a clean driving record and be able to safely operate a passenger van with wheelchair lift.
